Students in Construction Careers Academy should log their service hours using this form. Select the activity from the drop-down menu. If the service opportunity is not listed select the "Add Activity..." option and type the activity in the box provided. Service hours are periodically checked for accuracy. Students are required to provide verification of service hours as needed.
