COVID & Flu Vaccination Clinic @ Madison Elementary

Wednesday, October 13 2021

Volunteers will assist PUSD Health Services by directing traffic, registration, and observation for families as they make their way through through our neighborhood Vaccination clinic. Spanish speakers a plus! NOTE: Volunteers must be fully immunized for COVID-19.

Please arrive on time to get briefed on your duties and get specific on-site instructions.

1-2pm – Staff check-in and set-up
2-3:30pm – Registration and Vaccinations
3:30pm-4pm – Breakdown

Volunteers will have access to staff bathrooms and a designated break area.

WHERE:
Madison Elementary

PARKING:
Please park on nearby streets adhering to all parking signs.

REPORT TO:
The school parking lot corner of Madison and Buckeye and ask for Katia Ahmed, PUSD Health Services Coordinator. See "x" on attached map. She is your point person on site and will direct your volunteer service.

WHAT TO WEAR & BRING:
- Dress for comfort, mobility, and the weather.
- Wear closed-toe sneakers.
- Wear a hat to protect your face from the sun. Don't forget to put on sunscreen!
- Double mask yourself.
- Gloves and hand sanitizer will be provided.
- Water & snacks will be provided. Feel free to bring your own water as well.

SAFETY PROTOCOLS:

**All volunteers help with the understanding that they do so at their own risk. Anytime a person leaves their home, they increase their chance of exposure to COVID-19.**

We are asking for a specific number of volunteers. If you cannot make your shift, please let us know right away. Never volunteer if you are not feeling well. Cancel your shift.
